Are there any risks and side effects associated with fillers?
Hyaluronic acid dermal fillers are widely used in aesthetic treatments and are generally considered safe when performed by a trained practitioner. However, like any injectable treatments, fillers can have side effects and complications.
Most people experience only mild and temporary reactions such as swelling or bruising. In very rare cases, more serious complications may happen, such as vascular blockage, which may lead to skin damage if not treated quickly.
Other possible side effects include infection, small lumps, or filler migration. In some cases, using too much filler or having repeated treatments in a short period of time can increase the chance of the filler moving from the original injection site.
Over the last decade, reports of complications after aesthetic treatments have increased. This may partly be because more people are having cosmetic procedures than ever before.
Understanding the possible risks and knowing when to seek advice is an important part of safe filler treatment.
ARE COMPLICATIONS AFTER FILLERS COMMON?
Hyaluronic acid is a substance that can be naturally found in the body and is one of the safest fillers used in aesthetic medicine. However, some areas of the face carry a higher risk than others due to the location of important blood vessels.
These include the forehead, nose, temples, and around the eyes, and in these areas the most complications have been reported.
It is extremely important to quickly recognize any signs of complications and react fast, for example using hyaluronidase to dissolve the filler causing this complication.

WHAT ARE THE MILD SIDE EFFECTS AFTER HYALURONIC ACID FILLERS?
The most common reactions after filler treatment are mild and temporary.
These may include:
- Swelling
- Redness
- Tenderness at the injection site
- Minor bruising
These symptoms usually settle within a few days and do not normally require any treatment.
WHAT COMPLICATIONS CAN HAPPEN LATER?
Delayed reactions are rare but possible. Bacterial infections happen if people touch injection sites with unwashed hands, and hygiene guidelines are not followed after treatment. For example, using clean towels and bedding during the first days after treatment helps reduce this risk.
Bruising can also be delayed and show on the second or third day after the treatment.
One of the most serious complications is skin necrosis, which may happen if a vascular occlusion is not recognised and treated promptly.
WHAT IS A VASCULAR OCCLUSION AND WHAT ARE THE SIGNS?
A vascular occlusion is a rare complication that happens when filler blocks a blood vessel.
If recognised early, it can usually be treated effectively using hyaluronidase to dissolve the filler causing the blockage.
Possible signs include:
- Sudden whitening or blanching of the skin
- Increasing pain in the treated area
- Red, purple, or mottled skin patterns
- Skin that feels cold or firm to the touch
- Numbness or a pins-and-needles sensation
Any of these symptoms should be reported immediately to the practitioner.
CAN FILLER CAUSE A PERMANENT SKIN DISCOLOURATION?
Sometimes filler injected too close to the surface of the skin can cause what is known as the Tyndall effect.
This looks like bluish or greyish tint under the skin where the filler is visible. It happens when filler is placed too superficially and may be corrected with hyaluronidase.
CAN MY FILLER GET LUMPY?
In the first couple of weeks after filler treatment it is common to feel harder, and lumpy filler structure. This is temporary and settles as the filler integrates with the surrounding tissue.
Some people may develop lumps if
- Filler was injected too superficially
- Filler was distributed unevenly
- Nodules or granulomas form
If that happens small injections of hyaluronidase directly to the area can solve the problem by dissolving filler. Granulomas are not filler and cannot be dissolved.
INFECTIONS AFTER DERMAL FILLERS
Infections are very rare.
Signs of infection may include:
- Increasing redness and swelling
- Fever
- Yellow or green discharge
- Pain that worsens rather than improves
- Firm lumps or areas of hard tissue
CAN FILLER MIGRATE?
Filler migration is one of the more commonly discussed side effects. Migration means that filler moves away from the area where it was originally injected.
If too much filler has been injected under the eyes (tear trough) puffiness develops under the eyes. This can be reduced by hyaluronidase. It is important to assess the volume of filler in every area of the face, but especially under the eyes where the skin is much thinner.
The most common area treated for migration of the filler are lips. Usually excess of the filler moves outside the lip area (migrates), creating uneven shape of lips, lumpy and thicker area between the lips and nose or just spreading to the areas above or below the lips.
Sometimes migration is caused by the strong and overactive muscle around the lips. This is usually observed in clients with smaller and tighter lips, who had too much filler injected within a small space of time. We recommend treating lips gradually allowing the tissue to adapt to changes. We recommend to first relax the strong muscle before injecting filler, to allow the filler to settle uninterrupted.
